https://www.spaceflightnow.com/falcon9/001/f9guide.pdf Robust design margins Falcon9 is designed with the goal of carrying humans into space aboard the SpaceX Dragon capsule. This goal drives the initial design of Falcon9 through the incorporation of increased factors of safety (1.4 versus the traditional 1.25 for uncrewed flight). Payload customers using the Falcon9 can take advantage of this increased design robustness. The first and second stages are also designed to be recovered and reused, and therefore, must have significantly higher margins than anexpendable stage. This also provides a unique opportunity to examine recovered hardware and assess design and material selection in order to continually improve Falcon 9.
“Whether you load the crew first and then propellant, or propellant and then crew, either way the COPV issue has to be resolved,” Jett said. “While some people like to link those two issues, I think really you have to get comfortable with the COPV, then look at the hazards associated with the transient of having the crew on board during fueling.”

The tests showed that the Falcon 9’s helium pressure vessels can fail just from conditions created during helium loading. Both the temperature and the pressure of the helium can be problematic, according to SpaceX.
Now, SpaceX has been able to narrow down the cause even further and even replicate a similar failure through manipulating variables related to helium loading, particularly the temperature and pressure at which the fuel is pumped in.
Article Post: 11/1/2005 But tankage for super-cold liquid fuels -- liquid hydrogen, liquid oxygen (LOX) and others -- is still the purview of metals.
Why? Despite years of testing, concerns still exist about the potential for leaks, due to microcracking of traditional carbon/epoxy composite laminates at cryogenic temper-atures. Microcracks can occur in any laminate because of the difference between the axial and transverse coefficients of thermal expansion (CTE) in each ply, explains Brian Wilson, president of Wilson Composite Technologies (Folsom, Calif.).
Designing for containment of LOX poses particularly difficult problems. Any permea-tion leak poses a potent ignition-and-explosion hazard. There is unresolved debate in the industry as to whether LOX, an aggressive oxidizer, actually reacts with and degrades composite materials -- some, including Wilson, say that a cured composite is not affected. Composites also have a tendency to pop, flash or char if impacted mechanically while in contact with LOX, because the mechanical energy of the blow causes a rise in heat within the material, which can, in turn, cause auto-ignition.
